You must enroll by December 3, 2009, or you "May" LOSE Benefits.

Please Check Dependents for eligibility.

Please Check SSN for accuracy (Dependents as well as your own).

Please Check HMO's for new rates or changes in benefit levels.

Flexible Spending Accounts must be enrolled in every year.

Please Check the Vision Vendor/EYEMED for Provider Networks in your area before you enroll in Vision Plus. (Vision Plus premiums have increased).

Please Check all Provider Networks in your area before you make your decision as to which medical plan you want to enroll in.

CONSIDER PRE or POST TAX CONTRIBUTIONS.
If you decide you want to reimburse yourself the monthly premiums from
your Health Reimbursement Acct; you MUST ELECT POST TAX during
Open Enrollment.

Both the BC/BS PPO/Non Plan and the United HCN Plan now have the same
Deductible, Coinsurance and Out of Pocket maximums.

Preventative Care IS NOT covered out of network in either plan, but is
generally covered 100% In-Network if a preventative code is used for billing.

Print or request a CONFIRMATION OF COVERAGE at the end of
enrollment so that you have proof of enrollment.

Carefully review your Confirmation of Coverage for accuracy.
